- Have your physician complete a participant physical examination form and tuberculin skin test (PPD) form
- ACT TODAY! Seating is limited based on level of care
Admission is based on criteria of center to meet the needs of participants. In accordance with federal law and US department of Agriculture policy, this institution is prohibited from discriminating on the bases of race, color, national origin, sex, age or gender.
Payments can be made through qualified medical insurance plans and private pay. Let us tell you if you meet criteria for EDCD waiver, Veterans coverage or meet other requirements like Alzheimers Association grants and Full Time Scholarship grants.
